Platform for the empowerment and remote management of chronic patients.
• The main modules included are:
• Education and intervention: includes training sessions to learn how to manage one's own illness and exercises (e.g. psychoeducation exercise).
• Monitoring of subjective measures (entered by the patient and/or caregiver) and objective measures (collected automatically through external devices).
• Management of alerts and identification of baseline an dbehaviours and prediction of risk situations.
• Monitoring and clinical decision support.
• Interaction and support.
• Integration with health systems.
The platform is:
• Configurable: it allows to define different telemedicine programmes and assign them to specific patients. The same patient can be registered in different programmes. Each programme can include a different combination of education modules, exercises, collection of objective parameters (automatically via external devices), health questionnaires, etc.
• Scalable: allows the integration of new devices.
• Multi-device: accessible via PC, tablet or mobile.

- Patient monitoring module: pending activities associated with the patient in the telecare programme.
- Education module: provides information to patients and educators to increase adherence to treatment and identify early warning signs. It also allows for online psychoeducation exercises.
- Monitoring module: collects information recorded by both the patient and the caregiver. Access to historical records and generation of alarms and notifications.
- Support and interaction module: facilitates access to external content recommended by doctors and provides a space for interaction and exchange of experiences.
